Such happenings are considered and even labeled as “sentinel event(s)”.

They happen, and are rare.

The number often cited of 250,000 deaths per year through medical errors is an outrageous exaggeration. You do you and believe as you wish but that number is arrived at through cirque de sole level contortions.
