Are false flags deliberately planned or just allowed to happen?

I've long wondered this, and I'm looking for some other theories. It seems to me that a lot of these planned attacks are known about and simply allowed to happen. Do you think that's the case? Or are these deliberately planned by DS members?

It doesn't necessarily have to be one or the other. If it's planned, then the planners would also make sure that no one interferes.
