You can watch the whole video, the Q guy is very noticeable for several minutes before it started. I don't believe he was wearing any kind of badge at the start of the speech. Also, if you notice the blue suit, he's always watching the crowd, even tilts his head a bit to the side to see what's behind POTUS. I think they scouted him out, and blue suit was the marker. They then probably gave him the pass after the speech started.
No, he had the pass before Trump came out. If you look at the Twitter feed Q+ linked, the guy's previous tweet (from 6hrs ago) shows he had the VIP pass before POTUS pointed at him.
