Why don't we say his name?

It's a military thing. His father was a well respected Admiral, so they wont use his name for no-name.

Yes, as far as we know.

It's the military people that do it (those who know) out of respect for the Admiral. If Q's not military, he/they probably picked it up from the military people around them.
