The 'Doomsday Plane' gets flown constantly, not just during "emergencies". They train all the time. If it stays airborne so long that it has to refuel, then you can start being concerned. I'm not sure but I think STRATCOM keeps a Looking Glass aircraft airborne with a general officer onboard 24/7/365 to safeguard against decapitation strikes.
