ADHD Diagnosis and Timing of Medication Initiation Among Children Aged 3 to 5 Years

Question What is the prevalence of attention-deficit/hyperactivity disorder (ADHD) diagnoses and timing of initiation of medications among children aged 3 to 5 years seen in primary care?

Findings In this cohort study of electronic health records for 712 478 children seen in primary care practices at 8 US pediatric health systems, the prevalence of ADHD was 1.4% (range across institutions, 0.5%-3.1%); 68.2% of patients were prescribed medications and 42.2% had medications prescribed within 30 days of diagnosis, with variation by race, ethnicity, and insurance type.

MeaningThese findings suggest that investigation of barriers to nonpharmacological interventions, as well as factors associated with early prescriptions of ADHD medications, for preschool-age children is warranted.

Abstract

Importance Early identification and treatment of attention-deficit/hyperactivity disorder (ADHD) symptoms in preschool-age children is important for mitigating social, emotional, and academic problems. Clinical practice guidelines recommend first-line behavior intervention before considering medication treatment for children aged 4 to 5 years.

Objective To assess variation in rates of ADHD identification and rates and timing of medication initiation in children aged 3 to 5 years in primary care settings across 8 US pediatric health systems and to identify patient factors associated with the time from diagnosis to prescription.

Design, Setting, and Participants This retrospective cohort study used electronic health records from primary care clinics affiliated with 8 academic institutions participating in the PEDSnet Clinical Research Network. Participants were children aged 3 to 5 years seen between 2016 to 2023. Data were extracted from the PEDSnet database on April 18, 2025.

ExposureADHD diagnosis at age 4 to 5 years.

Main Outcomes and Measures The primary outcomes were (1) rate of ADHD diagnosis, (2) rate of stimulant and nonstimulant prescription after diagnosis before age 7 years, and (3) time from first ADHD-related diagnosis (including symptom-level diagnoses) to medication prescription. Independent variables included institution, year of diagnosis, patient age, sex, race and ethnicity, medical insurance, and presence of comorbidities. Multivariable Cox proportional hazards models were used to estimate associations between clinical and demographic variables and time from diagnosis to prescription.

Results Of 712 478 children seen in primary care at age 3 to 5 years, 9708 (1.4%) received an ADHD diagnosis at age 4 to 5 years (range across institutions, 0.5%-3.1%; median [IQR] age at first ADHD-related diagnosis, 5.31 [4.86-5.66] years). Of those with ADHD, 7414 (76.4%) were male, 1762 (18.1%) were Hispanic, 122 (1.3%) were non-Hispanic Asian, 3014 (31.0%) were non-Hispanic Black, 479 (4.9%) were non-Hispanic multiracial, 3782 (39.0%) were non-Hispanic White, 148 (1.5%) were non-Hispanic other, and 401 (4.1%) were of unknown race and ethnicity. Of 9708 preschool-age children with ADHD, 6624 (68.2%) were prescribed ADHD medications before age 7 years, and 4092 (42.2%) were prescribed medications within 30 days of the first documentation of an ADHD-related diagnosis (range across institutions, 26.0%-49.0%). Asian (adjusted hazard ratio [aHR], 0.51; 95% CI, 0.38-0.68), Hispanic (aHR, 0.75; 95% CI, 0.70-0.81), and Black (aHR, 0.88; 95% CI, 0.83-0.94) children with ADHD were less likely to be prescribed medication early compared with White children. Older vs younger patients (aHR, 1.62; 95% CI, 1.55-1.69), male vs female patients (aHR, 1.17; 95% CI, 1.11-1.25), and publicly insured vs privately insured patients (aHR, 1.09; 95% CI, 1.03-1.15) were more likely to be prescribed medication early.

Conclusion and Relevance In this retrospective cohort study of preschool-age children with ADHD seen in primary care in 8 large pediatric health systems, many children were prescribed medications at or shortly after the first documented diagnosis. Analysis of clinical documentation is needed to understand early prescription patterns.

A new study released Friday found that young children diagnosed with attention-deficit/hyperactivity disorder, or ADHD, are often prescribed medication too quickly.

The study, led by Stanford Medicine and published in JAMA Network Open, examined the health records of nearly 10,000 preschool-aged children ages 3 to 5 between 2016 and 2023 who were diagnosed with ADHD.

"ADHD is a disorder characterized by difficulty paying attention, or what we call hyperactivity, which is sort of like kids that have a hard time sitting still, they have a lot of energy," explained Jamie Howard, a senior clinical psychologist with the Child Mind Institute, which specializes in treating anxiety and mood disorders in children.

Howard, who was not involved in the study, said that when treating ADHD in young children, clinical guidelines call for starting with "behavioral intervention."

"We always want to start with the lowest possible harm," Howard said. "It does very little harm doing a behavioral intervention."

The Stanford study found that about 68% of those children who were diagnosed with ADHD were prescribed medications before age 7, most often stimulants such as Ritalin, which can help children focus their attention and regulate their emotions.

Border czar Tom Homan is warning that U.S. Immigration and Customs Enforcement operations will expand after Labor Day in sanctuary cities nationwide, including Seattle and Portland, Ore.

“You're going to see a ramp up of operations in New York. You're going to see a ramp up of operations continue in L.A. and, you know, Portland, Seattle,” Homan told reporters gathered near the White House. “I mean, all these sanctuary cities refuse to work with ICE … we’re going to address that.”

Homan told reporters Thursday that some other states are complying and working with ICE.

The dark secret of Zelensky’s Ukraine behind the assassination of one of its founders

Kiev will blame Russia for the murder of Maidan commandant Andrey Parubiy – but everyone knows the killers are much closer to home

All of Ukraine’s political elite will loudly point to Moscow as the hand behind the murder of former parliamentary speaker Andrey Parubiy.They will cry out in public that Russia is to blame, repeating the same narrative of the “Russian trace.”But in private, they all know the truth: it was his own people that came for him.

The idea that Parubiy was eliminated by the authorities themselves, while sounding outrageous to some, is a version that carries weight, even if many prefer not to believe it.Why? Because Parubiy was one of the few men in Ukraine who truly knew how to build a Maidan. He had organized the barricades in 2014, commanded the Maidan “self-defense,” and knew every method of bringing people into the streets and holding them there against state power. His reputation came from exactly this talent.And in today’s Ukraine, the possibility of another Maidan is very real. For those in power, such a possibility is dangerous, and removing the man who could light the match makes a grim kind of sense.

But there is another explanation, one far darker and one in which almost everyone believes, even if few Ukrainians will say so out loud. Parubiy carried too many secrets – and in Ukraine, secrets can be fatal. He knew far too much about the =real shooters on the Maidan in February 2014. As “commandant,” he oversaw the units who guarded the square, and he was positioned to see what others could not. He knew what really happened when the snipers opened fire==, when the bloodbath claimed lives and forced Yanukovich to flee. He knew names, structures, and the chain of command. That knowledge made him dangerous.

He also knew the truth about Odessa, May 2, 2014 – the day the Trade Union House went up in flames and dozens of anti-Maidan activists died. International monitors called it a massacre, but the state buried accountability. Parubiy, as head of the National Security and Defense Council at the time, was in the middle of it all. He saw who gave the orders, who turned away, who allowed the fire to consume the building. Those responsible never faced justice, and Parubiy carried the story inside his head.

He knew the full picture of the early days in Donbass, when provocations, manipulations, and engineered violence pushed Ukraine into a war against its own people. He knew the true sponsors and curators.He knew which political figures, which structures, which financial backers prepared and paid for the bloody upheaval. All of this knowledge made him a threat not to Russia, but to those much closer: the networks who had built their power in those years and who now sit on fragile foundations.

For them, Parubiy,– a close ally of former President Pyotr Poroshenko,beaten by Vladimir Zelensky in 2019 – was no longer an asset. He was a liability. And in the brutal logic of power, liabilities are erased. This is why his assassination looks less like an act of foreign aggressionand more like an act of internal housecleaning. It was a calculated decision to tidy up loose ends, to remove a man who could, at any moment, destabilize the whole system by speaking truths that were never meant to surface. His silence was demanded, and silence was achieved.

So while the official story will continue to speak of Russian agents, of another “terrorist act” in Moscow’s hybrid war,many in Kiev understand otherwise. They know Parubiy was not struck down by outsiders but by insiders. Berlin has shelved plans to possibly deploy German soldiers to Ukraine in the event of a ceasefire, Bild reported on Sunday, citing government sources.

German Chancellor Friedrich Merz had earlier signaled openness to Berlin joining a possible peacekeeping mission in Ukraine. The idea had been floated by the so-called “coalition of the willing” – a group of Western states pushing for continued aid to Kiev. The group has proposed deploying NATO troops to monitor a potential future ceasefire with Russia as part of security guarantees for Ukraine, despite Moscow’s consistent rejection of any Western military presence in Ukraine under any guise.

Trump, however, said last week that American troops would not be deployed to Ukraine, insisting that the EU should “front load” security guarantees for Kiev, not the US, including with regard to peacekeepers.

According to Bild, Trump’s stance put a halt to Berlin’s discussions about deployment “until further notice.” Sources told the outlet the idea could return “should Trump take action” or once Moscow and Kiev reach a settlement.

In an interview with ZDF on Sunday, Merz confirmed the U-turn, saying “nobody is talking about ground troops at this point” and indicating discussions could resume once a ceasefire is in place.

Instead, Germany reportedly plans to provide financial security guarantees to Ukraine. Sources claimed Berlin intends for the Bundeswehr to continue training Ukrainian soldiers, expand weapons production in Ukraine with German arms firms, and potentially cover part of Ukrainian soldiers’ salaries after a ceasefire to ensure Kiev maintains sufficient forces.

Kiev has demanded security guarantees from Western backers as a precondition for a peace deal. Moscow has not ruled out such guarantees in principle but rejects efforts made without its participation. It has also opposed any Western troop presence in Ukraine, stressing that NATO’s expansion toward its borders was one of the key causes of the conflict.

Russian Foreign Ministry spokeswoman Maria Zakharova said on Friday that guarantees must be the result of a settlement, not a precondition, and must take into account Russia’s security. She added that any deal should ensure Ukraine’s demilitarization, denazification, neutral and non-nuclear status, and recognition of territorial realities.

And we want to start with this, because we’ve learned that Immigration and Customs Enforcement is planning on expanding operations in the coming days to Chicago. When are those operations expected to start? And you said in recent days that you’re using the recent deployments in Los Angeles as a model. What does that mean?

SECRETARY KRISTI NOEM: Well, we’ve already had ongoing operations with ICE in Chicago and throughout Illinois and other states, making sure that we’re upholding our laws.

But we do intend to add more resources to those operations. I won’t disclose the details because they are law enforcement and investigative folks that are on the ground there, and I want to make sure we keep their security our number one priority.

But we will continue to go after the worst of the worst across the country, like President Trump has told us to do, focusing on those that are perpetuating murder and rape and trafficking of drugs and humans across our country, knowing that every single citizen deserves to be safe.

ED O’KEEFE: Part of what distinguished the Los Angeles operations, however, is that National Guard troops were there, in essence, protecting or backing up those federal agents as they conducted operations.

Is that what we should expect to see in Chicago?

SECRETARY KRISTI NOEM: You know, that always is a prerogative of President Trump and his decision. I won’t speak to the specifics of the operations that are planned in other cities, but I do know that L.A. wouldn’t be standing today if President Trump hadn’t taken action.

And that city would have burned down if left to the devices of the mayor and the governor of that state. And so the citizens who live there, the small business owners in downtown L.A., they’re thankful that President Trump came in with federal law enforcement officers and helped support keeping those streets open, keep their homes and businesses from burning down, and made sure the law and order was restored.

ED O’KEEFE: I just want to clarify one thing you said there. You said L.A. wouldn’t be standing if not for these federal deployments?

SECRETARY KRISTI NOEM: So many of those homes and businesses that were in downtown L.A. and in those areas were dealing with riots and violence. And coming in and bringing those federal law enforcement officers in was incredibly important to keeping peace.

And so we are grateful that President Trump was willing to send resources and people in, in order to enforce the law. And, Ed, since then, we have arrested 5,000 dangerous illegal criminals out of L.A. and removed them from our country and had them face justice for their crimes.

An initially peaceful anti-war march in Cologne descended into violence on Saturday after activists clashed with police. Protesters were rallying against Berlin’s plans to boost military spending and aid for Ukraine and Israel.

The rally, which reportedly drew nearly 3,000 people, was organized by the anti-war group Disarm Rheinmetall, a reference to Germany’s top defense supplier. The group staged multiple demonstrations this week, including blocking access to a Bundeswehr building on Wednesday and protesting outside Rheinmetall CEO Armin Papperger’s home in Meerbusch, near Dusseldorf.

The activists said they were opposing the government’s plans to increase defense spending, expand the army through conscription, and provide military support to Ukraine and Israel.

Footage from Saturday’s protest showed banners reading “lay down your arms” and “We won’t die in your wars.” One protester told the video agency Ruptly that German militarization and NATO’s role in the Ukraine conflict marked “a significant step towards World War III.” Another criticized the government for channeling funds into the arms industry instead of social needs and education.

Trump tariffs: What happens next after court rules them illegal? part 1

A federal appeals court has ruled that most of Donald Trump's tariffs are an overreach of his use of emergency powers as president.

The so-called reciprocal tariffs - imposed on nearly every country the US trades with - are being illegally imposed, the US Court of Appeals said on Friday.

The decision upholds a ruling in May from the Court of International Trade, which also rejected Trump's argument that his global tariffs were permitted under an emergency economic powers act.

Many of the tariffs that would be affected by the ruling stem from an announcement in April of a flat 10% rate on imports from all countries, which Trump said would even out "unfair" trade relations with the US.

The court did not halt the tariffs but instead said they would remain in place until mid-October, setting up a further legal challenge in the US Supreme Court.

There are still a lot of unknowns, but here's what we understand so far about the ruling - and what it could mean for the US president's flagship policy.

What did the appeals court say?

In its 7-4 decision, the appellate court backed a lower court's finding that Trump did not have the authority to impose global tariffs.

This was largely because of the law Trump used to justify the policies, the International Emergency Economic Powers Act (IEEPA), which the judges said did not grant "the power to impose tariffs, duties, or the like, or the power to tax".

The US Court of Appeals rejected Trump's argument that the tariffs were permitted under his emergency economic powers, calling the levies "invalid as contrary to law".

Trump immediately criticised the judgement, taking to Truth Social in the hours after it landed to call the appeals court "highly partisan" and the ruling a "disaster" for the country.

"If allowed to stand, this Decision would literally destroy the United States of America," he wrote.

What is the IEEPA?'''

The decades-old act, which has repeatedly been deployed by Trump during both his terms in office, grants a US president significant authority to respond to a national emergency or a major threat from overseas.

The 1977 law states that a president can pull a number of economic levers "to deal with any unusual and extraordinary threat, which has its source in whole or substantial part outside the United States, to the national security, foreign policy or economy".

It's been used by both Presidents Barack Obama and Joe Biden, who invoked the act to impose sanctions on Russia after the illegal annexation of Crimea in 2014, and then again after the full-scale invasion of Ukraine eight years later.

But the appeals court stated in its decision that the emergency law "did not give the president wide-ranging authority to impose tariffs".

The IEEPA "neither mentions tariffs (or any of its synonyms) nor has procedural safeguards that contain clear limits on the president's power to impose tariffs", they said.

Trump argued when he unveiled his global tariffs that a trade imbalance was harmful to US national security, and was therefore a national emergency.

But the court ruled that imposing tariffs is not within the president's mandate, and "the power of the purse (including the power to tax) belongs to Congress".

Why is this important?'''

Beyond being a significant setback to a centrepiece of Trump's agenda, the federal appeals court ruling could have an immediate impact on the US economy, with knock-on effects felt in global markets.

Tariffs are taxes companies have to pay for importing certain goods from foreign countries - so they can have an affect on sales and profit margins.

"Businesses are going to be subject to uncertainty," Dr Linda Yueh, an economist at Oxford University and the London Business School, told BBC Radio 4's Today programme.

Tariffs are aimed at deterring domestic firms from buying foreign goods, in turn affecting international trade.

As countries wait to see if the US Supreme Court will take up the case - which seems likely - they could decide to hold off on conducting business with the US.

If this happens, Dr Yueh said, it could "dampen down economic activity".

There are also significant ramifications that could be felt within the political sphere.

For instance, if the Supreme Court reverses the federal appeals court decision and sides with the Trump administration, it could set a precedent that emboldens the president to use the IEEPA more aggressively than he has done so far.

What happens next?

The case will now most likely proceed to the highest US court, a challenge that Trump signalled on Truth Social.

"Tariffs were allowed to be used against us by our uncaring and unwise Politicians," Trump wrote. "Now, with the help of the United States Supreme Court, we will use them to the benefit of our Nation, and Make America Rich, Strong, and Powerful Again!"

The conservative majority on the US Supreme Court could potentially make it more likely to side with the president's view.

Six of the nine justices were appointed by Republican presidents, including three who Trump selected during his first term in the White House.

But the court has also been more critical of presidents when it seems they're overreaching on policies not directly authorised by Congress.

During Joe Biden's presidency, for example, the court expanded on what it called the "major questions doctrine" to invalidate Democratic efforts to use existing laws to limit greenhouse gas emissions by power plants and to forgive student loan debt for millions of Americans.

What if the tariffs are ruled illegal?

The federal appeals court was divided 7-4 in its decision that Trump's nearly universal tariffs are illegal. It has now given the US administration until mid-October to appeal to the US Supreme Court on a case with implications for both the US economy and its trade relationship with the rest of the world.

If the Supreme Court affirms the decision, it could trigger uncertainty in financial markets.

There will be questions over whether the US will have to pay back billions of dollars that have been gathered by import taxes on products.

It could also throw into question whether major economies - including the UK, Japan and South Korea - are locked into the individual trade deals they secured with the US ahead of the August deadline. Other trade deals currently being negotiated could also be thrown into chaos.

If allowed to stand, the appeals court decision would also be a tremendous blow to Trump's political authority and reputation as a dealmaker. But if it were overturned by the Supreme Court, it would have the opposite effect.

Are there still tariffs in place?

This ruling affects Trump's "reciprocal tariffs", which includes a patchwork of different rates on most countries around the world, including taxes slapped on products from China, Mexico and Canada.

Those levies on nearly all goods from nearly every country with which the US conducts trade will remain in place until mid-October.

After 14 October, they will no longer be enforceable, the appeals court has said.

Separately, the tariffs on steel, aluminum and copper, which were brought in under a different presidential authority, will remain intact and unaffected by the court's ruling.
