A CBS News analysis of records for every hospice operating in Los Angeles County finds indications of fraud are growing.

At age 69, Lynn Ianni is a pickleball whiz, zipping from dinks to drives energetically. When she suffered an injury on the court two years ago, she sought physical therapy, and was surprised to learn her Medicare insurance wouldn’t cover it.

 

She was, according to Medicare records, dying and in hospice.

 

“They said, ‘you're in hospice.’ And I said, ‘what? What are you talking about?” Ianni said. “‘Are you kidding me? Do I look like I’m in hospice?’”

 

Ianni’s Medicare number had been stolen, and used by a company to fraudulently enroll her in hospice – specialized, compassionate care for terminal patients nearing the end of their lives. It was another example of fraud in the hospice industry, long a nationwide problem. But her case arose well after officials had promised to stamp it out in California, where the problem has been especially acute.

 

Medicare is federally administered, and hospices must be certified for reimbursements. But the state issues the licenses for hospices to operate.

 

Three years ago, California’s state auditor sounded the alarm that Los Angeles County had seen a 1,500% increase in hospice companies since 2010 – more than six times the national average relative to its elderly population.

 

Auditors estimated LA County hospices overbilled Medicare by $105 million in a single year. The report called out notable red flags – key warning signs of fraud:

 

Multiple hospices in one building

 

Geographic clustering

 

Low patient counts

 

High rates of terminally ill patients later discharged alive

 

Excessive billing

 

Staff shared across multiple companies

 

The state says it proceeded to investigate and revoke the licenses of 280 hospices.

 

But since then, the problem has continued to fester. CBS News examined the business and financial records of every hospice currently operating in LA County, applying the same indicators identified by the state. Indications of fraud have not stopped. In fact, they’ve grown.

 

The CBS News analysis reveals that over 700 of the roughly 1,800 hospices in LA County, trigger multiple red flags for fraud as defined by the state.

Scott MacFarlane, the CBS News justice correspondent most known for his extensive coverage of the January 6 Capitol insurrection, surprised colleagues on Monday morning by announcing his departure from the network, which he joined in late 2021.

“We’ve been told to aim our reporting at a particular part of the political spectrum,” Mary Walsh, a veteran CBS News producer, wrote in a memo late last month. “Honestly, I don’t know how to do that.”

 

Kim Harvey, the executive producer of the Evening News, pushed back on Walsh’s farewell comments in a follow-up note to her staff.

You may have already heard that Yamaha Motor Co. Ltd had decided to move its USA subsidiary out of Southern California. So is our headline somewhat overblown and possibly click-baity?

 

Maybe. But it’s also accurate.

 

Granted, the official news release from Yamaha Corporate in Tokyo last week was a little less dramatic: “Yamaha Motor Co., Ltd. has decided to relocate its U.S. group company, Yamaha Motor Corporation, U.S.A. (“YMUS”), currently located in Cypress, California, to Kennesaw, Georgia.”

 

So yes, Yamaha leaving its longtime Southern California offices in Orange Country after some 50-plus years is a big deal. The campus has served as the headquarters of YMUS since Yamaha Motor Co., Ltd. acquired the land in 1978 and established the office in 1979. But as is usually the case with these types of announcements, the backstory is a bit more nuanced, with multiple factors coming into play over decades, making the whole relocation angle seem like a totally rational business move once you zoom out.

First, the near-term stuff. Team Blue’s US headquarters is indeed pulling up stakes and heading to the other side of the country, but it’s not only swapping oranges for peaches, it’s selling the whole SoCal shebang, including land, offices, and warehouses. That makes a whole lotta sense, as the sprawling California campus—some 25 acres worth—had been reduced to housing mostly administrative and financial operations of late, and it was beginning to look like something of a white elephant for the firm.

 

Yamaha will begin the land sell-off in phases, starting from the end of 2026 and wrapping up by the end of 2028, thus shrinking its US real estate footprint considerably, and in turn swelling its bank account a good deal. In case you haven’t noticed, SoCal real estate isn’t exactly a bargain.

 

It’s also more than just a simple real estate transaction. As far as long-term strategies go, Yamaha USA has been making this shift for decades now; the subsidiary had already packed up and exported its marine division over to Georgia way back in 1999, followed a decade later by its motorsports business, which made the move in 2019. It’s probably better to think of the totality of all these actions rather than just the one move. It’s the latest step in a streamlining of operations, and this latest maneuver will consolidate all of Yamaha’s core US ops under one base, and likely improve manufacturing efficiency and logistical processes.

 

To be sure, there’s other fine print contributing to these “structural reforms,” as the corporate memo calls them, and we certainly can’t discount the prevailing market forces of the last few years. Especially this year, as the whipsawing action of US government-enacted tariffs contributed to a 30.4% drop in 2025 operating profit, according to Yamaha. So selling the California campus wasn’t a decision made in a vacuum, though it’s fair to say that current events may have hastened the move.

 

Once the sale of its assets is complete, Yamaha says it intends to lease back certain facilities in California to make sure no business operations are disrupted, and that the transition to Georgia is a smooth one.

 

Conspiracy theorists might point out that Zero’s, and now Yamaha’s recent relocations don’t bode well for other established two-wheel manufacturers in California, either. To which we say: Zero HQ’s move to the Netherlands last year was more the result of market forces (as in Europe being their fastest growing market, and the US going the other way) than any kind of California exodus. As for Honda, Kawasaki and Suzuki? They aren’t likely to be going anywhere else anytime soon. Then again, given the state of the headlines these days, who knows?

Cruising through X last week a weird story caught my eye: it reported that The Food and Drug Administration’s top drug regulator, Dr. Tracy Beth Hoeg, was trying to hire a “friend” who wants the FDA to add warnings to antidepressants about “unproven pregnancy risks.” The story makes several claims that are bewildering and appear to be fabricated. I sent several questions to AP’s global health editor Jonathan Fahey, but he did not respond to repeated requests to explain the article’s puzzling errors.

 

AP reporter Matthew Perrone later blocked me on X. I’ve pasted my email to Fahey at the bottom of this article.

 

The person AP’s Matthew Perrone identifies as a “friend” of FDA’s Hoeg is Dr. Adam Urato, chief of maternal-fetal medicine at Metro West Medical Center in Massachusetts.

 

One passage in the AP story stood out to me:

 

Within the agency, Hoeg’s close relationship with Urato is viewed as a clear conflict of interest that, under normal FDA standards, would result in her recusing herself from any work on the petition. But Hoeg is actively working to speed up the agency’s review of her friend’s proposal, according to the people familiar with the situation.

 

I have never seen the term “friend” defined as a “conflict of interest” by any federal agency. Nor have I run across “friend” defined as a “conflict of interest” in the peer-reviewed scientific literature. It’s a conflict of interest that doesn’t seem to exist.

 

And I happen to know quite a bit about conflicts of interest in science, because I’m an expert on the matter.

Australia’s Deepening Involvement in US War on Iran

 

Australia’s support for the U.S.-Israel war against Iran by hosting of a string of U.S. military bases and providing a ‘spy’ plane to defend the Gulf states is a critical contribution to the U.S. war machine, writes Peter Cronau.

 

Australia is doing what it has done for decades when the United States wages war in the Middle East.

 

Australia is providing intelligence architecture, intelligence collection, communication systems, military intelligence specialists, and the political cover that helps support the assault.

 

When the federal government announced on Monday that it will be sending a “surveillance” plane “in support of collective defence” of countries in the Persian Gulf, it will by necessity be effectively integrating and cooperating with the U.S.-Israel operation in the Gulf.

 

The line between ‘defensive’ and offensive’ is a blurry one at best. By defending the Gulf states from Iranian attacks, the Wedgetail will be mapping out the launch locations of those missiles and drones, to help track and intercept incoming strikes.

 

But the intelligence it gathers can also assist in U.S.-Israeli attacks on the identified launch sites and associated facilities. The E-7A Wedgetail is much more than a mere defensive aircraft and is described by the RAAF itself as “one of the most advanced airspace battle management capabilities in the world.” It says:

 

“The Wedgetail’s ability to coordinate a joint air, sea and land battle in real time significantly increases the effectiveness of the Australian Defence Force. The Wedgetail has been modified for the modern battlespace…. simultaneous tracking of airborne and maritime targets is made possible.”

 

Canberra says Australia is “not involved” because it is not dropping the bombs itself. That is the usual deception. But modern war is not just the aircraft over the target.

 

In a globally networked world, it is the satellite feed, the launch detection, the target support, the command post, the battle management platform, the submarine signals, the refuelling base, the overflight corridor, the embedded officers inside the coalition forces.

 

The country that provides those things is not outside the war, but is an intricate part of it.

 

The sprawling U.S. satellite base at Pine Gap in the centre of Australia just outside of Alice Springs is not a passive listening post. It is one of the U.S.’s most sophisticated satellite surveillance hubs, a source of real battlefield intelligence, and is integrated into U.S. nuclear warfighting.

 

Bringing overwhelming military surveillance technology to conflicts and wars, including the present war against Iran, is one of the main purposes for which the Joint Defence Facility Pine Gap (JDFPG) was built by the United States in Central Australia over fifty years ago.

 

And it has expanded exponentially since then. Initially hosting just two satellite dishes, the base has rapidly expanded in the past five years, and is now host to 45 satellite radomes and dishes.

 

An American former NSA analyst and veteran of Pine Gap verified what experts have long deduced, when he confirmed to Declassified Australia in November 2023 that Pine Gap was collecting intelligence data not only on the conflict in Gaza but also on “surrounding areas” in the Middle East. The intelligence gathered in Australia is passed to the U.S. who forwards much of it to Israel.

NEW YORK — The FBI found explosive materials in a Pennsylvania storage unit after two men were charged with bringing homemade bombs to a protest outside the home of New York City’s mayor, authorities said Tuesday.

 

Emir Balat, 18, and Ibrahim Kayumi, 19, told investigators after their arrests Saturday near Gracie Mansion in Manhattan that they were inspired by the Islamic State group,according to law enforcement officials and a criminal complaint. Both men are from the Philadelphia suburbs.

 

Much remained undisclosed about their motives, how much they planned and how they knew each other.Tests were being performed on some of the explosive devicesrecovered at the scene.

 

The FBI said Monday that it had conducted multiple searches in connection with the investigation.The police department in Middletown Township, Pennsylvania, said Tuesday that the FBI conducted controlled detonations of explosive materials found at a Public Storage facility in Langhorne, near where Balat’s family lives. Kayumi is from Newtown, about 4 miles (6.5 kilometers) north of Langhorne.

 

Residents “may have heard several loud bangs during the overnight hours,” the police department said in a Facebook post. “While the noise may have been startling to some residents, there was no danger to the public at any time.”

 

Balat’s lawyer has portrayed him as a confused teenager who didn’t know what he was doing.

 

Prosecutors, police and FBI officials say Balat and Kayumi drove to New York City on Saturday and joined a throng of counterprotesters at a small, anti-Muslim rally organized by the far-right Christian nationalist Jake Lang.

 

Journalists photographed Balat hurling a device, smoking with a lit fuse, that was later found to contain the explosive TATP. The object, which also contained nuts and bolts, extinguished itself without harming anyone.

 

Balat then dropped a second object near some police officers and tried to run, but was tackled and arrested, according to a court complaint

Balat and Kayumi were being held without bail after a court appearance Monday on chargesthat include attempting to provide material support to a foreign terrorist organization and using a weapon of mass destruction. They were not required to enter a plea.

 

Balat’s lawyer, Mehdi Essmidi, said outside court that his client, a high school senior, had “complicated stuff going on” in his personal life.

 

“I believe he’s 18 and he doesn’t have any idea what he’s doing,” Essmidi said. He added that he didn’t believe Balat and Kayumi had known each other for long.

 

New York Police Commissioner Jessica Tisch said Monday there were no indications that the attack was connected to the ongoing war in Iran. (The stupidest assessment ever!)

 

After Balat was arrested,police officers asked him whether he was aiming to accomplish something akin to the 2013 Boston Marathon bombing that killed three people.

 

“No, even bigger,”Balat replied, according to a criminal complaint.
