Transmission of SARS-CoV-2 Delta Variant Among Vaccinated Healthcare Workers, Vietnam

Abstract

Background: Data on breakthrough SARS-CoV-2 Delta variant infections are limited.

Methods: We studied breakthrough infections among healthcare workers of a major infectious diseases hospital in Vietnam. We collected demographics, vaccination history and results of PCR diagnosis alongside clinical data. We measured SARS-CoV-2 (neutralizing) antibodies at diagnosis, and at week 1, 2 and 3 after diagnosis. We sequenced the viruses using ARTIC protocol.

Findings: Between 11th–25th June 2021 (week 7–8 after dose 2), 69 healthcare workers were tested positive for SARS-CoV-2. 62 participated in the clinical study. 49 were (pre)symptomatic with one requiring oxygen supplementation. All recovered uneventfully. 23 complete-genome sequences were obtained. They all belonged to the Delta variant, and were phylogenetically distinct from the contemporary Delta variant sequences obtained from community transmission cases, suggestive of ongoing transmission between the workers. Viral loads of breakthrough Delta variant infection cases were 251 times higher than those of cases infected with old strains detected between March-April 2020. Time from diagnosis to PCR negative was 8–33 days (median: 21). Neutralizing antibody levels after vaccination and at diagnosis of the cases were lower than those in the matched uninfected controls. There was no correlation between vaccine-induced neutralizing antibody levels and viral loads or the development of symptoms.

Interpretation: Breakthrough Delta variant infections are associated with high viral loads, prolonged PCR positivity, and low levels of vaccine-induced neutralizing antibodies, explaining the transmission between the vaccinated people. Physical distancing measures remain critical to reduce SARS-CoV-2 Delta variant transmission.

The atrocities of Nazi medicine, as well as the conditions that made them possible, are even today a topic of heated debates among historians and bioethicists. Proponents of various positions often refer to the Nazi period in discussion of the ethics of research on human subjects. Two features of the Nazi period are crucial for understanding the specific forms of research on human beings undertaken then: the totalitarian political system and the broader paradigm of racial hygiene, which was not solely the result of the Nazi political system, but rather a social movement that drew on concepts created by understandings of contemporary biology. Its origins precede the beginnings of the Nazi party by more than two decades. But the autocratic political system and the programme of racial hygiene reinforced each other and contributed to specific questions to be addressed by the medical sciences, and to a setting in which no ethical or legal regulations existed. This combination of factors created the conditions for research to be undertaken that would not otherwise have been possible.

Many in the discipline, such as Fritz Lenz and Ernst Rüdin, hoped to see the practical application of the results of their scientific work, thereby contributing to rebuilding society according to the laws of biology. They also hoped to gain access to further resources to extend their research programmes. State and party institutions, in turn, were seeking scientific legitimation for their health and racial policies, such as the newly implemented sterilisation law “for the prevention of genetically diseased offspring”. The law allowed the forced sterilisation of those who supposedly had genetically determined disorders. Along with the later Law for the Protection of German Blood and German Honor, which purported the “racial inferiority” of Jews, considerable minorities of the population were defined as being “biologically” of minor value, with the implication that they lost most or all of their civil rights, and were easily available as “research material”.

Within the German army and the Nazi SS organisation, there were controversies over whether adding antibiotics or homoeopathic drugs to traditional surgery would improve wounded soldiers' chances of survival. By contrast, within the Allied armies, this question had already been settled in favour of antibiotic therapy. SS physicians organised trials of antibiotic and homoeopathic treatments in the concentration camps at Dachau and Ravensbrück. Healthy prisoners were given injections from the festering tissues of other inmates who had wound infections. In some people, small pieces of wood and glass were placed in open wounds, in order to mimic war injuries more realistically. The victims were then treated with homoeopathic preparations or various applications of sulfonamides; some received no therapy at all. About a third of the victims died. All these experiments followed a scientific logic that was outdated at the time, and which took no account whatever of the wellbeing of those involved in research. The surviving victims had irreversible physical damage and severe psychological trauma.

Myocarditis, a rare side effect, occurs mostly after the second dose. So in some countries, officialsare trying outsingle doses for children.

Even as parents in the United States wrestle with difficult questions over vaccinating their children against the coronavirus, families in other countries have been offered a novel option: giving children just one dose of the vaccine.

Officials in Hong Kong as well as in Britain, Norway and other countries have recommended a single dose of the Pfizer-BioNTech vaccine for children ages 12 and older — providing partial protection from the virus, but without the potential harms occasionally observed after two doses.

Health officials in those countries are particularly worried about increasing data suggesting that myocarditis, an inflammation of the heart, may be more common among adolescents and young adults after vaccination than had been thought.

The risk remains very small, and significant only after the second dose of an mRNA vaccine. But the numbers have changed the risk-benefit calculus in countries where new infections are mostly lower than in the United States.

Advisers to the Centers for Disease Control and Prevention reviewed data on myocarditis in June, and unanimously voted to recommend the vaccine for children ages 12 and older, saying the benefits far outweighed the risk.

Agency research has estimated that for every million vaccinated boys ages 12 to 17 in the United States, the shots might cause a maximum of 70 myocarditis cases, but they would prevent 5,700 infections, 215 hospitalizations and two deaths. Studies have also shown that the risk of heart problems after Covid-19 is much higher than after vaccination.

Myocarditis was among the concerns that led the Food and Drug Administration to ask vaccine makers this summer to increase the number of children in clinical trials. The issue is likely to be the focus of intense discussion when agency advisers meet later this month to review the evidence for vaccinations of children ages 5 to 11.

The latest analysis, which was published on Wednesday in The New England Journal of Medicine, found that the incidence of myocarditis after vaccination in Israel was highest among males aged 16 to 29. About 11 of every 100,000 males in that age group developed the condition a few days after being vaccinated, a rate higher than most earlier estimates. (The risk was negligible in females of any age.)

Of the 54 cases identified in the study, one was severe enough to require ventilation. Another patient with a history of heart disease died of an unknown cause soon after discharge from the hospital.

Of the 14 patients in the new study who showed heart abnormalities when they were admitted to the hospital, 10 still had some signs of problems when they were discharged. But when the patients were examined again a few weeks later, all five of those for whom results were available appeared to have fully recovered.

A second study, which was also published in the journal, found that boys between 16 and 19 years of age had the highest incidence of myocarditis after the second dose — nine times as high when compared with unvaccinated boys of the same age during the same time.

Health officials in other countries plan to revisit the one-dose strategy as more safety information becomes available, and they may choose to proceed with second shots. But the possibility of deferring the second jab has not received enough attention in the United States, said Dr. Walid Gellad, a drug safety expert at the University of Pittsburgh.

“In the U.S., people have not wanted to talk about it, for unclear reasons,” Dr. Gellad said. “Parents who are hesitant may appreciate the fact that the risk of side effects is actually much lower for one dose than it is for two doses.”

Serious side effects have primarily been seen in boys, so the dosing calculus should be different for boys and girls, he added.

It is too early to know whether myocarditis might permanently weaken the hearts of some people after vaccination, said Dr. Jeremy Brown, an expert in respiratory diseases at University College London and a member of Britain’s vaccine advisory group.

“That makes it very hard for us to make the absolute statement that it’s totally safe to give this vaccine,” Dr. Brown said. “We need some feel for what the long-term consequences of the myocarditis might be.”

With the Air Force and Space Force deadline of Nov. 2 less than four weeks away, the airmen and Guardians who have not yet been vaccinated will miss the mandate unless they opt to get the one-dose vaccine made by Johnson & Johnson within the next two weeks.

According to data obtained Wednesday by Military.com, 90% of the Army, 98% of the Navy, 95.4% of the Air Force and Space Force, and 85% of the Marine Corps have been partially or fully vaccinated.

For the Coast Guard, which falls under the Department of Homeland Security, 92% of the active duty force has received at least one dose, while 86% are fully vaccinated.

An official told Military.com that Coast Guard men and women have been ordered to "be fully vaccinated against COVID-19 as soon as operations allow, starting immediately."
